Declaration and Acknowledgement
By submitting this registration form, I hereby declare that all information provided herein is accurate and complete to the best of my knowledge. I understand that providing false or incomplete information may result in the refusal of this application or termination of camp participation without a refund.
I acknowledge that I have read, understood, and agree to abide by the camp's terms and conditions, including but not limited to the participation requirements, payment obligations, and cancellation policies as outlined by the Fayetteville Fury Soccer Camp team. I am aware that cancellations must be made 30 days before the camp start date to qualify for a full refund, minus any processing fees. Cancellations made after this period may not be eligible for a refund.
Furthermore, I understand that participation in the camp is at my own risk and the risk of my child. I hereby release and hold harmless the camp organizers, coaches, affiliates, and partners from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my child's participation in the camp.
I give permission for the Fayetteville Fury Soccer Camp staff to seek medical treatment for my child in case of injury, accident, or illness during the camp period if I am not immediately available to give consent. I agree to be responsible for any medical or other charges in connection with my child's attendance at the camp.
By signing below, I confirm my acceptance of these terms and provide consent for my child to participate in the Fayetteville Fury Soccer Camp.
